Blepharoplasty

Upper Blepharoplasty is one of the commonest procedures that Mr Banwell performs and it is associated with high patient satisfaction, rapid recovery and excellent results.

The aim of blepharoplasty, or reconstructive eyelid surgery, is to provide a more youthful, fresh (but natural) appearance to the upper and lower eyelids of male and female patients, assist in improving the drooping, sagging tired eye look and to remove redundant upper eyelid skin and fat that may be obscuring vision. In lower eyelid surgery we are able and to tighten any laxity of the lower lid, remove eyebags and remove any excess skin. Transconjunctival Blepharoplasty is another type of lower eye lid surgery that has been described.

Blepharoplasty surgery is usually performed as an inpatient uder a general anaesthetic for those who are not so squeamish, then the operation can be performed under a local anaesthetic. For upper eyelid (blepharoplasty) surgery an ellipse of skin and/or muscle and/or underlying fat is excised on average by external incisional means and the skin is stitched back together. For lower eyelids the skin is lifted from the lower lid by the surgeon and the fat removed.

The skin, following lifting, is then re-draped and the appropriate amount of excess skin removed. Any laxity within the lower lid can also be corrected by a tightening procedure (canthopexy).

The post-operative recovery is fairly straight-forward although swelling can persist for some weeks (more so with lower blepharoplasty). It is recommended to sleep on your back on pillows and avoid bending and lifting. We also recommend using witch-hazel eye pads to help reduce swelling and brusing; as the eyes may be sensitive after such operations, wearing sunglasses indoors and out can help reduce sensitivity.

TIME OFF WORK

Mr Banwell recommends lower eyelid surgeries are best performed under a general anaesthetic undertaken by a specialist anaesthetist. Patients may choose between a local or general anaesthetic for upper eyelid surgery.

Post-operatively it is expected that you will have significant bruising and swelling, but it wont be permanent. We encourage the use of cold packs to minimise this. It may take a couple of weeks to resolve. Our practice nurse will contact you daily after the operation for review, to ensure that you have no problems during your convalescence. Stitches will be removed after five days.

There are both general and specific risks and Mr Banwell will talk to you in detail regarding these possible complications and pass on tips. General risks include bruising and swelling, bleeding, wound infection, problems with general anaesthetics, abnormal scarring and keloid scarring. Specific risks include ectropion (eyelid pulled out), thickening of the scar beyond the outer corner of the eye, lagophthalmos (inability to fully close the upper eyelid), ptosis, scleral show, ‘wide-eye’ appearance, scarring of the cornea, blindness (extremely rare) diplopia (double vision-very rare), dry eye problems and eyelash hair loss, conjunctivits, grittiness and epiphora (watering).

I went to see Mr Banwell to get reassurance about the moles and blemishes which have been steadily growing since I was a young adult. 30 years ago we were not aware of the dangers of the sun and burning on holiday way an annual ritual. Thankfully we are all now aware that the sun damage can cause skin cancer and the changes in my moles mean I have a precautionary approach, particularly as older family members have had tumours removed. Paul knows exactly what he was looking for and studied each and every mole methodically. The two he highlighted for treatment were two of the least innocuous (in my opinion) which surprised me. Clearly to the trained eye they needed removal. I have since had an excision of one and due to have photo dynamic therapy treatment for the small red mark on my nose later this year. Without seeing Paul, this minor irritation would have been left and could possibly have created problems later in life. I advocate regular checks for anyone with moles and skin blemishes. The technology now is so impressive and it makes absolutely sense to take full advantage of Paul’s expertise.
